Background & Methods

WHO strongly recommends Kangaroo Mother Care (KMC) as a part of routine care for newborns to improve preterm birth outcomes. Ideally KMC should be practised for 24 hours, but due to lack of compliance it is practised for only 2–10 hours a day. The utilisation of a traditional cloth wrap has certain disadvantages that impede appropriate adherence to KMC. The KangaSling is a sturdy, hands-free, comfortable sling that can help improve compliance to kangaroo care. BEMPU is a device that can pick up hypothermia early and can alert the mother to provide KMC, hence improving compliance. This study assesses parent compliance to KMC with the KangaSling and BEMPU device in low-birth-weight babies between 1.5–2.5 kg compared to conventional kangaroo mother care. This three-arm RCT was conducted in the NICU and KMC ward of KLEH Dr. Prabhakar Kore Charitable Hospital. Ninety newborns were divided into: Group A (KangaSling + BEMPU), Group B (BEMPU + conventional cloth wrap), and Group C (conventional cloth wrap only).

Key Results

Outcome / MetricBEMPU GroupControl Groupp-value
KMC duration — hospital (hrs/day)Group A: highestGroup C: lowest<0.001
KMC duration — at home (hrs/day)Group A: highestGroup C: lowest<0.001
Weight gainGroup A: significantly betterGroup C: lowest<0.001
Length increaseGroup A: significantly betterGroup C: lowest<0.001
BEMPU alarms triggeredGroup A: fewestGroup B: more<0.001
  • 1Babies in the KangaSling + BEMPU group (Group A) had significantly longer KMC duration both in hospital and at home compared to conventional care.
  • 2Group A babies achieved significantly better weight gain and length increase than both Groups B and C.
  • 3Fewer BEMPU hypothermia alarms in Group A, reflecting more continuous skin-to-skin contact.
  • 4Time to full breastfeeding was shortest in Group A, though not statistically significant.
  • 5The combination of KangaSling and BEMPU device promoted the best overall neonatal outcomes.

Why This Matters

This is the first three-arm RCT to directly compare the combined effect of the KangaSling and BEMPU device against BEMPU alone and conventional care. The study shows that combining ergonomic KMC support with real-time temperature monitoring produces better compliance and growth outcomes than either intervention alone.
